McArthur Aged Care is seeking an experienced and passionate Registered Nurse to join a respected not-for-profit aged care provider in Northern Tasmania as a Clinical Care Partner.

About the Role
As a Clinical Care Partner, you will provide clinical oversight, assessment and care management for clients receiving services under the Support at Home program.
Your responsibilities will include
- Managing a caseload of clients with clinical needs, ensuring safe, high-quality, person-centred care.
- Conducting comprehensive clinical assessments and developing, implementing and reviewing individualised care plans.
- Monitoring changes in clients' health and coordinating timely, appropriate clinical interventions.
- Providing clinical guidance, education and support to Care Partners and community care teams.
- Working collaboratively with clients, families, support workers and other healthcare professionals.
- Ensuring care delivery aligns with current aged care standards, clinical governance requirements and best-practice principles.
- Providing clinical nursing support in the community when required.
About You
To be considered, you will need:
- Current AHPRA registration as a Registered Nurse - essential.
- Previous experience in community aged care - essential.
- Strong clinical assessment, care planning and case management skills.
- An understanding of Home Care Packages and/or the Support at Home program.
- Excellent communication skills and the ability to build positive relationships with clients, families and colleagues.
- Confidence working independently and managing competing clinical priorities.
- Sound IT skills and experience using electronic care management systems.
- A current driver's licence and willingness to undertake community-based client visits.
