Seeking an Experienced Nurse (EEN) – Aged Care and Health Assessment Services – Immediate start

We are seeking an enthusiastic, compassionate, and suitably qualified Enrolled Nurse (EN/EEN) to support the delivery of health assessments and comprehensive medical assessments for older Australians.

The successful applicant will work collaboratively with general practitioners and other healthcare professionals to promote preventative healthcare, support healthy ageing, and improve health outcomes for patients.

Key Responsibilities

- Assist with conducting health assessments for eligible patients.
- Support GPs in completing comprehensive medical assessments for aged care residents and older patients.
- Collect patient medical histories, record vital signs, and document relevant clinical information.
- Assist with medication reconciliation, care planning, and identifying potential health risks.
- Support the coordination of referrals, follow-up appointments, and ongoing patient care.
- Maintain accurate clinical records and ensure compliance with relevant clinical standards and privacy requirements.
- Provide compassionate, respectful,



and patient-centred care.

Essential Requirements

- Current registration with the Nursing and Midwifery Board of Australia (AHPRA) as an Enrolled Nurse.
- Experience or an interest in aged care, general practice, or community healthcare.
- Excellent communication, organisational, and documentation skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- A commitment to delivering high-quality, person-centred care.
