Key Responsibilities
Depending on the organisation and position, responsibilities may include:

- Managing and coordinating a portfolio of Home Care clients.
- Developing, implementing and reviewing individual care plans.
- Conducting client assessments and identifying changing care needs.
- Managing client budgets and coordinating services under the Support at Home program.
- Building positive relationships with clients, families and healthcare professionals.
- Ensuring services comply with aged care legislation and quality standards.
- Monitoring client wellbeing and coordinating appropriate support services.
- Maintaining accurate documentation, case notes and client records.
- Supporting older Australians to remain independent in their own homes.

About You
We are interested in hearing from individuals with:





- Experience in Home Care, Community Aged Care, Care Coordination or Case Management.
- An understanding of Home Care Packages, Support at Home or CHSP.
- Experience coordinating care services, conducting assessments or developing care plans.
- Excellent communication and relationship-building skills.
- Strong organisational skills and the ability to manage competing priorities.
- A genuine passion for delivering person-centred care.
