About the Role

As a Residential Care Case Manager at Junction Support Services, you will provide trauma-informed, therapeutic and person-centred case management to children and young people living in residential care. Through building trusted relationships and identifying strengths, you will empower young people to achieve their goals and develop the skills needed for independence and future success.

- Strengthen connections to family, culture, identity and community, supporting a sense of belonging and resilience.
- Lead care team collaboration, maintain quality documentation and support successful transitions toward independence and future goals.

This role is about more than case management. It is about creating opportunities, building resilience and supporting children and young people to thrive in a protected, stable and nurturing environment.

About You

You will bring:

- Relevant qualifications in Community Services, Social Work, Welfare, Psychology or a related discipline, and/or extensive experience in case management within child, youth and family services.
