AMITA Health Care Group is seeking a caring, reliable and motivated Disability Support Worker to provide individualised support to an NDIS participant.

The role will primarily involve community access, meal preparation and assistance with everyday activities, with a focus on supporting the participant to maintain independence, choice and control.

Key responsibilities include:

- Supporting the participant with community access and social activities
- Assisting with shopping, appointments and other community-based activities
- Meal planning and preparation
- Supporting the participant with daily living tasks as required
- Encouraging independence and participation in activities of the participant’s choice
- Providing companionship and positive social engagement
- Supporting the participant to work towards their NDIS goals
- Providing transport where required
- Maintaining professional boundaries and respecting the participant’s privacy, dignity and choices
- Completing progress notes and other required documentation
- Following the participant’s support plan, risk assessments and organisational policies

To be considered, applicants should have:

- Previous disability or community support experience preferred




- Certificate III in Individual Support, Disability, Community Services or similar is desirable
- Current NDIS Worker Screening Check
- Working With Children Check, where required
- Current First Aid and CPR
- Valid Australian driver’s licence
- Reliable vehicle with appropriate insurance
- Good communication and interpersonal skills
- Ability to work independently and reliably
- A respectful, patient and person-centred approach
