City to Country Care is looking for compassionate, reliable and dedicated NDIS Support Workers to join our growing team across the Port Macquarie district!

About the Role
As an NDIS Support Worker, you will provide person-centred support to participants in their homes and within the community, helping them live meaningful, independent and fulfilling lives.
Key Responsibilities
- Provide assistance with daily living activities, personal care and household tasks.
- Support participants to access the community, attend appointments and participate in social activities.
- Encourage independence, skill development and achievement of individual goals.
- Follow individual support plans and behaviour support strategies where required.
- Maintain accurate shift notes, documentation and incident reports.
- Build positive, respectful and skilled relationships with participants,
families and the wider support team.
- Promote participants' dignity, choice, privacy and safety at all times.
About You
We are seeking individuals who are caring, patient, dependable and committed to providing high-quality support.
Essential Requirements
- Certificate III in Individual Support, Disability Support or a relevant equivalent qualification (or higher).
- Current Australian driver's licence.
- Reliable, roadworthy vehicle with appropriate insurance.
- Current First Aid and CPR certificates, or willingness to obtain them if accepted for the role.
- NDIS Worker Screening Check.
- Working rights in Australia.
- Strong communication, interpersonal and documentation skills.
- Flexibility to work a variety of shifts, including weekdays, evenings, weekends and active or inactive overnight shifts where required.
Pay: $31.00 – $57.35 per hour
Expected hours: 8.0 – 30.0 per week
