The role is a 6-month fixed-term full time position, primarily on-site at St Stephens Hospital with extensive community visits. Compensation ranges from $59.43 to $60.70 per hour plus salary packaging, allowances and travel reimbursements. You will provide person-centred home-based care, develop care plans, coordinate with multidisciplinary teams and ensure safety and
