About the role
You'll travel to client's homes across the Gosford area, delivering hands on clinical care assessments. Your visits will focus on dementia and diabetes care, wound management, and general clinical review, helping clients stay safe and independent at home.

Duties
- Deliver skilled in-home clinical care
- Assess and redress wounds
- Manage diabetes care and insulin
- Support clients living with dementia
- Complete clinical assessments each visit

Skills & Experience

- Current, unrestricted AHPRA registration
-Home care or community nursing background
- Current police check & NDIS worker screening (or willing to obtain)




- Current first aid and CPR
- Drivers Licence and reliable, insured vehicle
