About the Role
The Community Field Nurse is a front-line position acting as a representative of My Guardian providing the services offered by the company.
Their duties are not limited to those listed as the position has a wide scope of duties at times dependent on the needs of the company.
This position suits someone who enjoys structure, works well under pressure, and thrives in a quick-paced care environment.
Key Responsibilities
Conduct comprehensive home assessments and design personalised care plans that truly meet each client's needs.
Lead, mentor and empower care teams, providing expert nursing guidance and ensuring seamless service delivery.
Deliver hands-on clinical care, including wound management, medication administration and dementia assessments.
Monitor client wellbeing, promptly identifying and reporting any changes in health or behaviour.
Coordinate holistic, multidisciplinary care by liaising with hospitals, doctors and allied health professionals.
Maintain accurate, compliant documentation aligned with policy,



funding and regulatory standards.
Build strong client relationships while fostering a high-performing, customer-focused culture.
Support operational excellence through collaboration with My Aged Care, NDIS and internal administration teams.
About You
Current and valid AHPRA Registration
Ability to work independently
Excellent communication and listening skills
Compassionate, patient and caring nature
High level of organisational and time management skills
Competent using Google Docs and able to learn business systems quickly
Must have experience with care systems.
Sound computer skills in Office and Excel (or similar)
Able to work in a fast-paced environment.
Driver's license with access to own vehicle
