As a Support Worker with Connection Care Services, you will provide individualised support based on each participant's goals, preferences, support plan and agreed arrangements.Depending on the participant and shift, your responsibilities may include:assisting with daily living activitiespersonal care and hygiene supportsocial and community participationtransport to appointments, shopping and activitiesmeal preparation and shopping supportdomestic assistance and household routinessupporting participants to develop everyday skills and independencemedication support where appropriately trained and authorisedfollowing participant support plans, routines and communication preferencesmaintaining accurate shift notes and relevant documentationcommunicating professionally with participants, families, carers and other members of the support networkidentifying and reporting incidents, risks or changes in support needs in accordance with organisational proceduresYou will be expected to provide support in a way that promotes participant choice, control, dignity, safety and independence.The exact duties,
