Metro South Palliative Care Service is seeking a Palliative Care Social Worker to deliver compassionate psychosocial assessment and support to adults in palliative care across community and inpatient settings, including Canossa and Wynnum Manly Palliative Care Units.

You will provide comprehensive psychosocial assessments, educational support, and collaborate with multidisciplinary teams to coordinate care and ensure protected transitions.
