The role involves comprehensive assessments, goal‑directed treatment plans, and coordinating discharge planning, including home modifications and equipment prescriptions.

You will work in a multidisciplinary team, support junior staff, and contribute to service improvements while delivering patient‑centred care in a energetic public health setting.
