About the Role
/n As a Home Care Support Worker, you will provide practical, social and personal support to clients in their homes, helping them maintain independence, dignity and quality of life. /n This is a community-based role where no two days are the same. You will be part of a team that values connection, reliability and making a real difference.

Key Responsibilities
/n
· Provide personal care including showering, dressing, grooming and toileting /n
· Assist with domestic support such as cleaning, laundry and meal preparation /n
· Support community access including appointments, shopping and social outings /n
· Deliver companionship and emotional support /n
· Promote independence, choice and dignity /n
· Observe and report changes in client wellbeing /n
· Maintain accurate documentation and communication with the care team About You/n
· Passionate about supporting older people to live well at home /n
· Reliable, compassionate and respectful /n




· Strong communication and interpersonal skills /n
· Ability to work independently and as part of a team /n
· Flexible and adaptable across different client needs Essential Requirements/n
· Certificate III in Individual Support (Aged Care, Home & Community or equivalent) /n
· Current First Aid and CPR /n
· National Police Check (or willingness to obtain) /n
· NDIS Worker screening /n
· Valid driver’s licence and reliable vehicle /n
· Smartphone for rostering and communication systems
