Uniting NSW.ACT is seeking a dedicated Case Manager to lead the delivery of person-centred home and community care services in Tamworth. You will coordinate care planning, act as the central client contact, and ensure services align with funding and client needs.

This full-time, fixed-term role runs until 11 April 2027 and requires travel between Uniting sites and client locations, solid collaboration, and a commitment to quality and safety in care delivery.
