Life Without Barriers is seeking a dedicated youth services skilled to provide case management for young people aged 15 and over preparing to leave care. You will develop goal-focused plans, coordinate housing, education and health supports, and empower independence within a supportive team.
Regular travel will be required across the service area, with opportunities to work autonomously while contributing to meaningful youth outcomes.
