Are you an experienced Case Manager, Support Coordinator, or disability professional looking to take the next step in your career?
At Alya, we're looking for a proactive, compassionate and organised Participant Support Officer (PSO) to join our growing team, managing a caseload of NDIS participants across the Moreton Bay, Brisbane and Sunshine Coast regions.
This is a fantastic opportunity to make a meaningful difference in the lives of people with disabilities, while enjoying a flexible hybrid working arrangement and a supportive team environment.
About Alya
Alya is a registered NDIS provider specialising in psychosocial, complex health and physical support needs.
We provide individualised disability supports across Southeast Queensland, with a strong focus on building independence, achieving personal goals and empowering participants to live fulfilling lives.
Our frontline Support Workers, known as Mentors, provide more than day-to-day assistance. They build meaningful relationships and support participants to develop skills, achieve goals and improve their quality of life.
About the Role
As a Participant Support Officer (Case Manager), you'll be responsible for overseeing a caseload of NDIS participants across Moreton Bay, Sunshine Coast and Brisbane, ensuring they receive safe, consistent and high-quality supports.
You'll be the key connection between participants, their families, support workers and external stakeholders, coordinating services and ensuring each participant's individual needs and goals remain at the centre of their supports.
Your key responsibilities will include:
- Managing and overseeing a caseload of NDIS participants across Moreton Bay and the Sunshine Coast.
- Building and maintaining strong relationships with participants, families, Support Coordinators and allied health professionals.
- Conducting regular participant visits, reviews and wellbeing check-ins.
- Coordinating participant supports and working collaboratively with rostering teams to ensure appropriate Mentor allocation.
- Monitoring service quality, participant satisfaction and progress towards individual goals.
- Supporting the development, implementation and review of participant support plans and risk assessments.
- Responding proactively to participant concerns, incidents and changes in support needs.
- Maintaining accurate case management records and ensuring compliance with NDIS Practice Standards and organisational policies.
- Working collaboratively with Alya's clinical, operational and People & Culture teams to deliver high-quality participant outcomes.
About You
We're looking for someone who is confident, personable and genuinely passionate about supporting people with disabilities.
Ideally, you'll bring:
- Previous experience in Case Management, Support Coordination or a similar role within the disability, community services or healthcare sectors.
- A background as a Registered Nurse (RN), Enrolled Nurse (EN) or Assistant in Nursing (AIN) is highly regarded, but not essential.
- A sound understanding of the NDIS and person-centred disability supports.
- Strong communication, relationship-building and problem-solving skills.
- Excellent organisational skills and the ability to manage competing priorities.
- Confidence working independently while remaining connected to a collaborative team.
- A proactive approach to participant wellbeing, risk management and service delivery.
We also welcome applications from experienced Disability Support Workers who are ready to progress into a case management role.
Essential Requirements
Applicants must have:
- Current NDIS Worker Screening Clearance (Yellow Card).
- Current Queensland Working with Children Check (Blue Card).
- Current Australian Driver's Licence.
- Reliable vehicle with comprehensive car insurance.
- Full Australian working rights.
- Availability to travel regularly throughout the Sunshine Coast, Moreton Bay and Brisbane for participant and stakeholder visits.
- Ability to attend our Stones Corner, Brisbane office three days per week.
Hybrid Working Arrangement
This role offers a hybrid working arrangement consisting of:
- 3 days per week: Based at Alya's head office in Stones Corner, Brisbane.
- Remaining working days: Flexibility to work from home and conduct participant visits across Moreton Bay and the Sunshine Coast.
