- Supporting participants with diverse needs, including neurological conditions, autism spectrum disorder (ASD), psychosocial disability and other complex circumstances.
- Maintaining accurate and timely case notes, service agreements, reports and client records.

About You – Essential Requirements

You must have at least 12 months of Support Coordination experience and be confident managing an existing caseload independently from day one.

We are looking for someone who:

- Has practical NDIS Support Coordination experience and can hit the ground running.
- Has a strong understanding of current NDIS legislation, guidelines, processes and recent reforms.
- Can manage complex participant needs, competing priorities and stakeholder relationships.
- Is organised, reliable, professional and committed to meeting billable hour targets.

- Has excellent written and verbal communication skills.
- Can work independently while contributing positively to a small team.
- Holds a Bachelor’s degree in a community-related field, preferably, or at minimum a relevant Diploma.

Required checks and qualifications:

- Current NDIS Worker Screening Check.
- Current First Aid Certificate.
- Current driver’s licence and reliable vehicle.

Previous experience in the community services sector is highly regarded.
