to join our Support at Home team and make a real difference in the lives of our
clients.
About
the role
As
a Clinical Care Partner, you will deliver high-quality, person-centred,
culturally safe clinical care management services to older people living at
home. Working in alignment with the Support at Home program and our clinical
governance framework, you will play a vital role in ensuring clients maintain
independence, wellbeing, dignity, and safety in ways that reflect their
culture, identity, and preferences.
You
will:
- Undertake comprehensive clinical assessments, care
planning, and service coordination.
- Provide evidence-based clinical risk assessment,
education, and guidance for clients with complex needs and chronic health
conditions.
- Offer clinical oversight and support to non-clinical
Care Partners, ensuring early identification and management of clinical
risks and deterioration.
- Ensure services are delivered in line with the
Strengthened Aged Care Quality Standards.
About
you
You
are a dedicated and compassionate health professional who thrives on building
meaningful connections with clients and colleagues. With a solid clinical
background, you are confident in providing oversight and coaching, while
ensuring the highest standards of care.
You
will bring:
- Current registration with AHPRA as a Registered Nurse
(or equivalent clinical qualification).
- Experience in community care, aged care, or chronic
disease management.
- Strong clinical assessment and care planning skills.
- The ability to work collaboratively across
multidisciplinary teams.
- A commitment to person-centred, culturally safe care.
