Your next employer:
Join a Home Care provider that supports older people to remain independent in their own homes. The team works closely with clients, families, support workers and health professionals to make sure care remains appropriate as needs change.
Are you a Registered Nurse who is calm, experienced and comfortable taking ownership of client care? Someone who can build rapport, make sound decisions and be a steady point of contact for both clients and the team? The stars might be aligning!
The opportunity:
You'll take the lead on care for clients with more complex home care needs, primarily those receiving higher-level packages (Levels 3 and 4). You'll make sure the right services are in place and adjust care arrangements as circumstances change.
It's a role that draws on your nursing judgement, but most of your time will be spent coordinating care, completing assessments and building relationships with clients and families rather than delivering hands-on nursing visits. You'll be part of a service supporting approximately 100 clients overall, with your focus on those requiring more complex care and coordination.
- Welcome and onboard new clients, completing assessments and arranging appropriate services.
- Develop and review individual care plans, including annual reviews and updates when clients' needs change.
- Monitor client package budgets, ensuring services align with their needs, preferences and available funding.
- Visit clients in their homes, building strong relationships with clients and families.
- Work closely with allied health professionals and external providers to coordinate care.
- Provide guidance and support to support workers, particularly when clients' needs or care arrangements change.
- Undertake occasional hands-on clinical nursing duties when required.
- Maintain accurate and up-to-date client records and documentation.
What makes this great:
- Permanent, full time position paying approximately $110,000 per annum.
- Work vehicle, fuel card and laptop provided.
- Monday to Friday hours, generally 9.00am–5.00pm, with flexibility around start and finish times.
- A chance to build lasting relationships with clients and families, rather than only seeing them for individual nursing visits.
- A predominantly care coordination role, with a good balance of client contact, assessments, care planning and occasional clinical nursing.
About you:
This is an ideal opportunity for an experienced Care Partner, Care Manager or Community Aged Care RN. You know how to assess a client’s needs, write a useful care plan and have an honest, reassuring conversation with a family when things change.
- Current AHPRA registration as a Registered Nurse.
- Experience in home care, client assessments, care planning and reviews.
- Good understanding of home care packages, individual client budgets and funded services.
- Confidence supporting care workers and working collaboratively with allied health professionals and external providers.
- Sound clinical judgement and willingness to provide occasional hands-on nursing care.
- Strong communication, organisational and documentation skills, including confidence using electronic client records.
- A current driver's licence and willingness to visit clients in their homes.
- Current First Aid and CPR certification, or willingness to obtain them.
