Mercy Community in Rockhampton is seeking an experienced Care Partner to join our Community Aged Care Services team on a permanent full time basis. The role requires travel within the Capricorn Coast Region and delivers care management to help older people live safely at home.

You will work in a rights-based, person-centred framework, collaborating with participants, their supporters and interdisciplinary teams.
