Just Better Care Gippsland is seeking a Care Partner to join our local Gippsland team in Rosedale. You will manage a caseload of Support at Home participants, develop and review individualised care plans with participants and families, and coordinate with the wider care team.
You'll work within NDIS and TAC guidelines, maintain accurate records in AlayaCare, and build solid relationships with GPs and allied health.
