Each health services is seeking a Registered Nurse to join the Health@Home team in Ferntree Gully. You will coordinate care for clients living with chronic and complex health conditions, helping them stay connected to community support and thrive at home.

You will collaborate with clients, carers, GPs and providers to develop tailored care plans, deliver health education, monitor progress and ensure thorough documentation. This role emphasizes independence, self-management and proactive care.
