Service Integration Coordinator - Connected Care
As Service Integration Coordinator, you’ll help establish a new statewide mental health service by building relationships and referral pathways that connect people to the right support at the right time. You’ll map health and community services, connect with local organisations, support community engagement and maintain the Connected Care Support Catalogue. This is a practical, relationship-focused role with variety and the opportunity to help shape how a new ‘no wrong door’ service works across Tasmania.
About Connected Care
Connected Care is a statewide mental health service delivered by Wellways in partnership with Stride Mental Health and Clarity Health Care and commissioned by Primary Health Tasmania. The service supports adults aged 18 years and over to access free, coordinated mental health and wellbeing support, using a stepped-care approach that responds to their individual needs and goals. A multidisciplinary team brings together clinical,
psychosocial and peer expertise to support people to access the right care and move between levels of support as their needs change.

- Support engagement with diverse and underserved communities
- Contribute to resources, reporting and the evaluation of referral pathways

The role can be based in Tasmania’s South with regular travel across the region and some travel elsewhere in Tasmania.
What you bring
You’ll bring experience building strong working relationships across health, mental health or community services, with the ability to connect people and organisations and support the development of practical referral pathways. You’ll have an understanding of community development and recovery-oriented,
trauma-informed and strengths-based approaches, along with knowledge of the broader services that support people's mental health and wellbeing.
