GV Health in regional Victoria is seeking a Project Manager for the Transition Care Program on a fixed-term, part time basis. You will lead a quality improvement project examining engagement, referral activity and occupancy across TCP services, collect and analyse data, and work with internal and external stakeholders to implement improved service models.

The role supports service redesign and planning to ensure residential TCP services stay client-centred, financially sustainable and aligned
