About the Role:

Renal care is moving closer to home for communities in Adelaide's north — and this role sits at the heart of that shift. As a Senior Exercise Physiologist embedded within the Northern Adelaide Local Health Network's expanding renal program, you'll deliver a specialist visiting service from GP+ Elizabeth, helping dialysis patients build strength, function, and quality of life without leaving their community. This is a part-time, ongoing role, sitting within the Intermediate Care Service at GP+ Elizabeth.

The Allied Health Chronic Disease Intermediate Care Service (ICS) comprises of Exercise Physiology, Dietetics, Physiotherapy, Occupational Therapy, Podiatry, Social Work and Interdisciplinary Complex Self-Management support services, operating as an intermediate service between hospital admission and community supports. Our team works collaboratively, and in alignment with other services within NALHN Clinical Divisions, as well as local Aboriginal Health Services, with a focus on optimising functional and health outcomes and reducing hospital admissions for clients with Diabetes and Chronic Cardiac and Respiratory Conditions. At ICs we aim to meet the needs of the client and “Together, we support people to build confidence in managing their health and wellbeing,

so they can achieve what matters most.”

Your Role in the Renal Team

Working as part of a dedicated multi-disciplinary renal team, you'll provide evidence-based exercise physiology services to patients receiving dialysis through NALHN's extended northern renal footprint. This is a part-time, ongoing role, sitting within the Intermediate Care Service at GP+ Elizabeth.

- Assess, plan, and deliver individualised exercise interventions for patients with chronic kidney disease and end-stage renal failure
- Collaborate closely with renal physicians, nurses, pharmacists, dietitians, podiatrists, social workers and other professional health colleagues to deliver coordinated, patient-centred care
- Monitor patient progress and adjust programs in response to clinical status, dialysis schedules, and individual goals
- Contribute to service development as this visiting model matures across the northern community

What You Bring

- Sound understanding of chronic disease management and functional rehabilitation
- Strong understanding of the physiological considerations specific to patients on haemodialysis or peritoneal dialysis
- Experience contributing to multi-disciplinary team environments
- Confidence working in a visiting or outreach service model with a degree of autonomous practice
- Current driver's licence and capacity to work across offsite NALHN locations

To be eligible for this position, you must have an appropriate Degree or equivalent qualification, usually a bachelor's or postgraduate degree in clinical exercise physiology and eligible for accreditation as an Accredited Exercise Physiologist from Exercise Sports Science Australia (ESSA).
