The Royal Melbourne Hospital invites applications for a Progression of Care Navigator on a part time basis at the Royal Park Campus in Melbourne's Parkville precinct. You will work within the ward's multidisciplinary team to coordinate discharge planning and improve patient flow.

You will be registered Division 1 RN or Allied Health, with at least five years in sub-acute care and strong communication with patients, families and clinical staff.
