The Mala’la Health Service seeks a Chronic Disease ITC Program Care Coordinator to lead care planning for clients with complex chronic conditions in Maningrida and surrounding homelands. The role is Darwin-based with FIFO: 2 weeks in Maningrida and 2 weeks in Darwin, with accommodation and flights subsidised.
Applicants must hold current AHPRA RN registration, BLS, Medicines Management, and have at least 5 years nursing experience, plus experience in remote Aboriginal communities.
