The Princess Alexandra Hospital (PAH) in Brisbane, part of Queensland Health, is seeking a Nurse Navigator to lead the CAREPACT program. You will coordinate person-centred care across hospitals, aged care facilities, GPs and community services to improve access and outcomes for residents. You will bring extensive nursing experience, gerontology expertise and a drive to reduce fragmentation while empowering residents and families through education and shared decision-making.
