Our client is seeking a compassionate and organised Care Coordinator to join their team in Sydney's Northern Beaches, supporting older Australians and individuals living with disabilities, dementia and ongoing care needs.

Key Responsibilities

- Conduct client assessments and develop person-centred care plans.
- Coordinate home care services and manage ongoing client needs.
- Liaise with families, support workers and healthcare professionals.
- Monitor service delivery and respond to changes in client circumstances.
- Maintain accurate documentation and ensure quality care.

About You
- Diploma of Nursing, Community Services or equivalent preferred.




- Experience in aged care, disability support, home care or care coordination.
- Robust communication, organisation and relationship-building skills.
- A genuine passion for delivering high-quality, person-centred care.
