Implementation Scientist
- Neighbourhood Health Hubs Primary & Community Care Services – Ryde NSW Implementation Scientist – Neighbourhood Health Hubs PCCS is leading innovative, community-driven approaches to integrated care across health and social systems. We are seeking an experienced Implementation Scientist – Neighbourhood Health Hubs to support the development, implementation and ongoing adaptation of Neighbourhood Health Hubs. This role is ideal for someone who brings a robust implementation science lens, understands complex systems, and can work alongside communities, providers and local institutions to create sustainable change.
About the Role
Neighbourhood Health Hubs are designed to help communities realise their own health by strengthening the relationships, local assets, shared practices and practical pathways that enable people to connect, contribute, seek support, access care and build healthier, safer and more meaningful lives. This is not a conventional service implementation role. The goal is not simply to roll out a model, create referral pathways or coordinate providers. The goal is to understand what enables change in real neighbourhoods, identify what helps or hinders adoption, and support local people, community groups, health services, social services, councils, communities, businesses and informal leaders to work together as one system for the community. As the Implementation Scientist, you will apply implementation science, community activation and systems-thinking approaches to help local teams design, test, adapt and sustain Neighbourhood Health Hubs in ways that reflect local context and community need. You will work across formal and informal systems, including general practice, hospitals, NGOs, peer-led services, local councils,
Aboriginal and Torres Strait Islander community leadership, multicultural communities, lived experience leaders, neighbourhood centres, libraries, clubs, schools, churches, mosques, temples, community groups, volunteers, businesses and residents. You will help PCCS and local partners understand what works, for whom, in what circumstances and why. You will support practical measurement, shared learning and continuous improvement so that Neighbourhood Health Hubs are not only established, but embedded, sustained and improved by the communities and systems around them. Why This Role Matters People do not live their lives inside service systems. They live in families, streets, clubs, workplaces, schools, faith communities, cultural groups, online spaces and neighbourhoods. Health is shaped by whether people are connected, safe, valued, active, supported, able to contribute, able to access care, and able to find meaning and belonging in daily life. Neighbourhood Health Hubs bring together clinical care, social care, community life and lived experience so people are not left to navigate fragmented services alone. This role matters because communities already hold many of the assets that generate health. The task is to help those assets become more visible, connected, inclusive and useful, while ensuring people can still access formal care when they need it. Using an implementation science approach, this role will help identify barriers and enablers,
support adoption of new ways of working, test and adapt approaches in real time, and generate learning that can inform future neighbourhood-based models of care. The purpose is not to build another standalone service. The purpose is to help the neighbourhood work as one system for health.
Key Responsibilities
Lead the implementation of Neighbourhood Health Hubs using implementation science, systems-thinking and community-led approaches. Identify local barriers, enablers and readiness factors that influence the adoption and sustainability of Neighbourhood Health Hubs. Bring together residents, community members, GPs, service providers, peer workers, councils, volunteers and local leaders around practical shared action. Support small local teams to understand community needs, activate local strengths, solve access barriers and test practical solutions. Work across formal and informal systems to strengthen connections between clinical care, social care, community assets and lived experience-led supports. Translate the Neighbourhood Health Hub vision into practical implementation approaches that can be tested, adapted and embedded in diverse local contexts. Use data, stories, community feedback and simple measures to understand what is working, what is not working and what needs to change. Support local partners to move from discussion to action while maintaining shared purpose and momentum. Build capability in local teams so communities and providers can continue improving the system themselves. Capture implementation learning, including what works, for whom, under what conditions and why. Contribute to the sustainability, adaptation and potential spread of Neighbourhood Health Hubs acros
