St Vincent’s Health Australia is seeking a Grade 3 Clinical Social Worker – Residential Aged Care for SVHM RACFs (Berengarra, Prague House and Auburn House). The role is permanent part time 0.5 EFT (20 hours/week) located in Kew/Hawthorn East, Melbourne.

You will deliver advanced psychosocial assessment and care planning for residents and families, provide clinical leadership, supervise staff and students, and work across campus with multidisciplinary teams to improve care and service
