St Vincent’s Health Australia is seeking a part time (32 hours per fortnight) Nurse Practitioner/Clinician based in Fitzroy to provide clinical assessment and management for residents of RACFs and the community, reducing avoidable ED presentations. You will offer clinical consultancy to RACF and community staff and support discharge planning with care coordination.

The role requires current AHPRA registration, postgraduate training in aged care or acute/critical care, and strong assessment and
