About the Opportunity

As a Clinical Care Coordinator, you will be responsible for coordinating and supporting patient care journeys from referral through to discharge.

Working within a multidisciplinary team environment, you will assess referrals, coordinate treatment pathways, monitor patient progress, manage clinical escalations and ensure patients receive safe, high-quality care through virtual and home-based care models.

Importantly, you won't just be joining an established service.This is a rare opportunity to join a flagship service during its growth and implementation phase.
