About the Program
Country Care Connect Service provides multidisciplinary, community‑based support to individuals experiencing severe and complex mental health, psychosocial, behavioural, and care coordination needs across rural and regional communities.
The program delivers intensive, recovery‑oriented support for patients who may experience barriers accessing traditional mental health services due to geographic isolation, complexity of presentation, psychosocial vulnerability, service fragmentation, or co‑occurring challenges.
Based in a GP practice, the service model includes outreach, multidisciplinary collaboration, care coordination, and integrated care with local health, social, and community services to improve outcomes for individuals with complex support needs.
Local Community and Practice
