MARS Healthcare is currently seeking an experienced Social Worker to join a highly regarded regional Victorian health service, working within a supportive and welcoming Community Allied Health team.

About the Role

Working as part of the Community Allied Health team, you will provide social work services across three key programs:

- Transition Care Program (TCP): Supporting patients transitioning from hospital into the community or longer-term care arrangements.

- Rehabilitation in the Home (RITH): Providing psychosocial assessments,



care coordination and support to patients receiving rehabilitation in their homes.

- Geriatric Evaluation and Management (GEM): Supporting older adults within an outpatient rehabilitation program to maximise independence and wellbeing.

You'll work collaboratively with a multidisciplinary team to support patients and their families, facilitate discharge planning and coordinate appropriate community services.

About You

- Tertiary qualification in Social Work

- Eligibility for AASW membership

- Previous experience in community health, rehabilitation, aged care or hospital social work

- Strong assessment, discharge planning and case management skills

- Ability to work independently and collaboratively within a multidisciplinary team

- Available to commence 2nd November 2026
