About the Role

We’re looking for a Case Manager to join our Home Care team on a full-time basis.

In this role, you’ll manage your own caseload of Home Care clients, building trusted relationships with clients and their families and making sure the care and support they receive continues to meet their individual needs, goals and preferences.

You’ll be responsible for coordinating care, reviewing and updating care plans, monitoring services and managing client package budgets to ensure funding is being used appropriately. You’ll also work closely with direct care staff and external providers, advocating for clients and helping them navigate their available services and supports.

About You

In addition, you will have:

- Demonstrated experience in Case Management, Care Coordination or a similar role within Home Care, Aged Care, Community Services or Health
- Robust understanding of person-centred care and Consumer Directed Care principles
- Experience with client assessments, care planning and coordinating services to meet individual needs
- Strong communication, organisation and relationship-building skills,



with the ability to manage a diverse caseload
- Strong computer literacy, including experience using client management systems and Microsoft Office applications
- Experience managing client budgets, funding and resources would be highly regarded
- Current Australian Driver’s Licence
- Reliable vehicle with current registration and insurance
- National Police Clearance (or willingness to obtain)
