About the role

As a Social Worker at Sunrise Way, you will work collaboratively with residents, colleagues, and external service providers to support individuals throughout their rehabilitation journey. You will undertake individual case management, psychosocial assessment, NDIS applications, and therapeutic interventions to help residents address the underlying factors contributing to their substance use, strengthen their resilience, and develop practical strategies for maintaining recovery. A large portion of the role will be supporting residents as they prepare for life beyond residential rehabilitation, helping them establish appropriate community connections, access relevant services, and work towards sustainable independence and long-term recovery.

Working within a multidisciplinary team, you will contribute to a therapeutic community that encourages personal responsibility, positive relationships, emotional growth, healthy routines, and meaningful participation in everyday life.

Key responsibilities

- Provide person-centered social work assessment, case management, and NDIS Applications to support residents participating in Sunrise Way's residential rehabilitation programs.
- Develop, implement, monitor, and review individualised case plans that support recovery goals, wellbeing, personal development, and community reintegration.
- Support residents to explore the social, emotional, psychological, family, and environmental factors associated with substance use and recovery.
- Apply trauma-informed, strengths-based, recovery-oriented, and evidence-informed approaches to practice.
- Assist residents to develop emotional regulation, coping strategies, communication skills, healthy routines, personal accountability, and effective problem-solving skills.
- Support residents to rebuild family relationships, strengthen social connections, and develop healthier interpersonal boundaries.




- Facilitate access to appropriate external services, including mental health professionals, medical practitioners, housing services, employment and education providers, financial and legal assistance, and community-based supports.
- Work collaboratively with counsellors, psychologists, support workers, health professionals, and other relevant stakeholders to promote coordinated care.
- Support residents to engage with appropriate peer-support networks, community activities, and ongoing recovery resources.
- Contribute to discharge planning, transition arrangements, and continuing care to support sustainable recovery beyond the residential program.

About you

- A recognised tertiary qualification in social work or related field.
- Relevant experience in social work, case management, psychosocial support, or a related field.
- A demonstrated understanding of alcohol and other drug dependence, mental health, trauma, and the social factors that can affect recovery.
- Knowledge of person-centred, strengths-based, trauma-informed, and recovery-oriented approaches.
- Strong written documentation, report-writing, communication, and interpersonal skills.
- The ability to maintain confidentiality, exercise professional judgement, and work within appropriate ethical and qualified boundaries.
- Current Australian working rights.
- A current Australian driver license.
- Any mandatory screening clearances, checks, or certifications required for the position.
