VMCH in Melbourne is seeking a compassionate Client Care Consultant for our Care at Home team. You will guide prospective clients and families through consultations, assess needs and help them navigate care options to live well at home.
You will convert consultations into signed service agreements while ensuring every interaction is skilled, supportive and client-centred. You’ll develop care plans, explain funding and service options, and coordinate handovers with intake, admissions and
