Rural Lifestyle Options Australia Ltd is seeking a Care Partner to support older Australians with Support at Home. You will welcome new clients, develop personalised care plans and coordinate services with a dedicated team to reflect individual needs and goals.

Working with Care Manager and coordinators, you will conduct assessments, reviews, and home visits, and ensure secure, person-centred care. A current driver’s licence and NDIS Worker Screening clearance are required.
