You will deliver and develop tailored Care Plans for each client and oversee ongoing assessment of needs within Home Care.
The role requires a Certificate IV in Aged Care with at least 2 years of experience, knowledge of Home Care Packages, CHSP and Aged Care Standards.
You will travel between sites and support staff to meet funding and service expectations.
