cohealth is seeking a Community Health Nurse for a part time, ongoing role (0.4 FTE – 2 days: Monday and Wednesday) at our Fitzroy clinic in Melbourne.
You will deliver culturally safe, client-centred nursing care, develop tailored care plans, and connect clients with services across sectors.
You will work within a collaborative MDT to support diverse clients, provide health education, and empower patients and families to improve wellbeing.
