The Role

You'll be the person who helps turn a client's needs and goals into a practical plan for living well at home. Through phone and video assessments, you'll ask the right questions, identify risks, recommend suitable services and develop care plans and budgets that give clients more choice and control. It's a role that combines meaningful conversations, critical thinking and practical problem solving, with the opportunity to make a difference every day.

What You'll Do

- Conduct personalised client assessments, understanding individual needs, goals and preferences
- Develop tailored care plans and budgets aligned with Support at Home funding
- Identify potential risks, recommend appropriate supports and arrange referrals where needed
- Guide clients and families through care options, helping them make informed decisions
- Manage a busy schedule of assessments while maintaining accurate records and documentation
- Collaborate with internal teams to deliver safe, consistent and high quality care

What You'll Bring

- Strong communication skills and a natural ability to connect with people




- Experience in customer service, aged care, NDIS, healthcare or a similar environment
- Confidence asking questions, solving problems and navigating sensitive conversations
- Excellent organisational skills and the ability to manage competing priorities
- Confidence using technology and learning new systems
- A proactive attitude, sound judgement and a genuine interest in helping others
