Coolamon Shire Council is seeking a Permanent Part time Clinical Care Partner to coordinate care for clients with complex needs and provide nursing oversight within the Community Care team. You will conduct clinical assessments, develop care plans, and support non-clinical staff to deliver safe, person-centred home-based care.

With a Bachelor of Nursing and current AHPRA registration, you will lead clinical governance, monitor health status, and collaborate with health professionals to ensure
