Yea & District Memorial Hospital in Victoria is seeking a compassionate My Care Clinic Intake Officer / Service Navigator to join our team. In this frontline role you will manage administrative coordination, triage referrals, and navigate service pathways to ensure patients access the right care.

You will work with autonomy to handle complex referrals, identify barriers to care, coordinate multidisciplinary access, and support achievement of clinic access targets.
