Recent Home Care in NSW seeks a Full-Time Community Registered Nurse to deliver evidence‑based assessments, complex care, and care coordination across Home Care Packages and Support at Home programs. You will lead clinical practice, liaise with GPs, allied health, and hospitals, and uphold the Aged Care Quality Standards.

You will conduct in‑home assessments, develop tailored care plans, manage wound care, medications, and chronic disease monitoring, while mentoring Enrolled Nurses and PCWs as part
