Institute for Urban Indigenous Health Ltd invites applications for a Mob Link Social Health Care Coordinator. You will provide culturally protected, person-centred, short-term coordination for Aboriginal and Torres Strait Islander people facing barriers to health and social services.

Work with clients to identify priorities, connect them with housing, mental health, and other supports, and facilitate warm referrals.
