Austin Health is seeking a TCP Project Manager to lead the development and implementation of a Nurse Practitioner-led model of care within the TCP Community Program. You will work closely with the Nurse Practitioner and Community Aged Care Services Manager to shape future nursing models and support relationships with Associated Providers.

Responsibilities include designing clear nursing resources, escalation pathways, and a framework to support in-house models, with emphasis on team-oriented
