This fixed term, part time role (64 hours per fortnight) is based at Graham Street and supports patients in Palliative Care, Rehabilitation and Geriatric Evaluation and Management within a multidisciplinary team.
You will provide practical and emotional support to patients and families, manage a clinical caseload, and contribute to the broader social work service.
