Moran Aged Care is seeking a dedicated Care Planning Coordinator to join the Clinical Leadership team at Moran Home in Kellyville. This Monday to Friday on-site role offers the chance to work closely with clinical leaders, influence resident outcomes and contribute to best-practice care in a supportive setting.
Reporting to the Care Manager, you will oversee the overall clinical assessment program, support RNs to complete assessments, and help tailor care plans to each resident’s needs.
