Western Health in Melbourne’s west is seeking a Case Manager to join our Community Mental Health Team. You will provide recovery-oriented care, develop and review care plans, assess risk, and work with a multidisciplinary team to promote safety and wellbeing for people with complex mental health needs.

Applicants will bring at least two years of clinical mental health experience, solid communication skills, and a current Victorian driver’s licence.
